I need to implement a Performance Evaluation for workers by their Bosses

So, I need to personalize an specific Survey adding the Worker name & the Boss name in order to identify clearly who evaluates to whom

Any Idea ?

The context is a boss has to evaluate his employees

Human resources area defines which evaluates to whom, so bosses receives a survey link

The idea is when the survey is open, the first page show some boss and employee facts

Is there any way to pass parameters in the url ?

ie. http://surveys/hhrr/survsurvey.aspx/Supervisor?boss=John&emp=Peter&empposition=Supervisor

After get the parameters and put them in fields

I have to say a boss has to evaluate more tan one worker, so We need to send a mail  with a list of links with people to be evaluated
